Impaction Types

Understanding Impacted Wisdom Teeth

When there isn't enough space for wisdom teeth to emerge properly, they become “impacted.” Here are the common types:

Moderate

Vertical Impaction

Tooth is straight but trapped below gum

Complex

Horizontal Impaction

Tooth lying completely sideways

Most common

Mesial Impaction

Tooth angled toward front of mouth

Complex

Distal Impaction

Tooth angled toward back of mouth

Treatment Approach

Simple vs. Surgical Extraction: Which Do You Need?

Depending on the position of your tooth, your dentist will recommend one of two approaches:

1

Simple Extraction

This is performed on wisdom teeth that have fully erupted and are visible. The dentist uses an elevator to gently lift the tooth and forceps to remove it. No incision is usually required, and recovery is faster.

2

Surgical Extraction

This is necessary for impacted teeth trapped under the gum or bone. A small incision is made in the gum to access the tooth. In some cases, the tooth may be sectioned (divided) into smaller pieces to make removal easier and minimize bone removal. This technique reduces trauma to the jaw and speeds up healing.

FAQs

Wisdom Tooth Extraction FAQs

Everything you need to know about adult braces in London.

At our clinic, wisdom tooth extractions start from £125 for members (from £250 standard) for simple cases, with surgical extraction from £325 for members (from £650 standard). Your initial assessment costs £30 for new patients or £47.50 for members, with X-rays included. We provide an exact quote after X-ray assessment. No surprises.

The extraction itself is performed under local anaesthetic so you will feel pressure, not pain. Post-operative discomfort is normal and well managed with paracetamol and ibuprofen. Most patients find it far less unpleasant than they expected.

A simple extraction takes 15–30 minutes. A surgical extraction for an impacted tooth typically takes 30–60 minutes. The full appointment including assessment and aftercare advice is usually 60–90 minutes.

For a simple extraction, 3–7 days of gradually improving discomfort is typical. You can usually return to work the next day. Surgical removal of impacted teeth has a longer recovery of 5–10 days.

No. Wisdom teeth that are fully erupted, cleanable and not causing problems are usually best left alone. Extraction is indicated when there is recurrent infection, decay, orthodontic impact or risk to adjacent teeth.

An impacted wisdom tooth is one that cannot fully emerge because there is not enough space in the jaw. It may be partially or fully trapped beneath the gum or bone. Impacted teeth are more prone to infection and may require surgical extraction.

Dry socket (alveolar osteitis) occurs when the blood clot that forms after extraction is dislodged or dissolves before the socket heals. It causes significant pain around day 3–4. To prevent it: don't smoke for 72 hours, avoid straws, don't rinse vigorously for 24 hours, and follow all aftercare instructions.
